Personalized Transition Planning: After a hospital stay or health event, we help veterans navigate what comes next. We coordinate with discharge planners, and family to assess needs and quickly connect you to community resources, benefits, support services, and follow-up care — creating a clear roadmap for a safer, smoother transition home and beyond.
Temporary Free In-Home Assistance: Community Bridge of Southwest Florida delivers expert, free Healthcare Navigation and Outreach Coordination for local veterans and seniors. Beyond addressing Social Determinants of Health, VA benefits, and specialized support for hospital discharges or Alzheimer’s/dementia care, we actively protect independence through key bridge services. We coordinate accessible transportation—such as LeeTran Passport—to ensure non-driving veterans maintain mobility and access appointments. Additionally, while long-term VA-funded homecare is pending, we coordinate temporary, free in-home assistance through trusted agency caregivers. From daily living aid (bathing, dressing, meal prep) to companionship and medication reminders, this vital bridge keeps veterans safe and independent at home without delay. .
VA Advocacy & Navigation: Our experienced advocates serve as expert guides through the often-complex VA system, helping veterans and families "speak VA language," to avoid common denials or delays to expedite approvals for long-term resources like VA Homemaker/Home Health Aide (H/HHA) care, which provides ongoing assistance with daily activities to promote independence; non-skilled home health; or respite for caregivers. Our persistent, knowledgeable support maximizes benefits you've earned, ensuring sustained access to the care needed for a stable, dignified life at home.
Holistic Support: We go beyond basic care by connecting veterans to a full spectrum of local Lee County and Southwest Florida resources tailored to whole-person well-being. This includes linkages to the Lee County Department of Human and Veteran Services for benefits assistance, community networks like Mission United or United Way programs, caregiver respite options (through VA or local groups), peer support groups, mental health services, nutrition programs, transportation aid, and integrative wellness opportunities. We emphasize integration with the VA's Whole Health program—helping veterans explore personalized health plans focused on what matters most to them (through assessments, self-care practices like mindfulness or movement, and classes)—to support physical recovery, emotional resilience, social connection, and overall quality of life. Our goal: comprehensive, coordinated support that addresses body, mind, and spirit for lasting health and independence.
