Community Bridge of Southwest Florida delivers unparalleled expertise in Healthcare Navigation and Outreach Coordination, backed by deep roots serving veterans and seniors across Southwest Florida. We address key Social Determinants of Health (SDOH)—including housing, food security, transportation, and homecare—by seamlessly connecting individuals with trusted community organizations and vital resources, all at no cost.
Beyond standard navigation, we provide specialized, urgent support for veterans transitioning out of hospitals or rehabilitation facilities, ensuring safe, fully coordinated hospital-to-home discharges. We also offer dedicated guidance for veterans living with Alzheimer’s or dementia and their families, helping them navigate complex memory care needs and caregiver support. From securing VA benefits to maintaining safety and independence at home.
HOME BRIDGE Program Overview
Community Bridge of SWFL served 4 veterans (average age 73.4) between April and July 2026. qualified for the HOME BRIDGE program.
Target Population
Veterans enrolled in VA healthcare who are:
Preparing for discharge from a hospital or rehabilitation center to home
Living at home while managing Alzheimer's disease or dementia
Key Impacts & Operational Strengths
More With Less: Through efficient workflows, resources originally scoped for 1 veteran were leveraged to serve multiple veterans within limited grant funding.
Rapid VA Care Authorization: HOME BRIDGE secured VA Homemaker-Home Health Aide (H-HHA) approvals in an average of 4 days post-agreement — and 2 days on average once connected with the veteran's VA PACT social worker. The standard VA timeline is ∼10 days after a PCP order.
Holistic, No-Cost Integration: Veterans received care at no personal expense and were connected to:
Skilled nursing care
Food assistance and transportation
Library access programs
VA Service Officers for claims and benefits advocacy
