Community Bridge of SWFL is a dedicated nonprofit serving residents of Southwest Florida, with a special focus on Lee County.
We bridge critical gaps by connecting underserved seniors, veterans, and those facing isolation or limited access to essential services—with compassionate healthcare, vital community resources, and strong local partnerships.

Bridging Veterans to Whole Health
Community Bridge of Southwest Florida exists to support veterans by placing the whole person at the center of care. We connect veterans to trusted community resources that promote physical health, mental well‑being, independence, and social connection. By building strong partnerships and closing gaps in services, Community Bridge empowers veterans to improve their quality of life, strengthen their support systems, and thrive within their community—not just survive.
